Support for Blue Badges/Bus Pass/DLA forms

If you require an 'evidence of a hidden disability' form to be completed, for a Blue Badge application, please contact the Headteacher. You will need to supply your Case Reference number from the application in order for this to be processed. The form will be returned to you as a PDF document to attach to your application.

Should you require proof of address and disability for a bus pass, please contact the Headteacher who will sort this for you. 

Should you require school evidence for a DLA application, please contact the class teacher who will sort this. You will need to send the rest of the application, with all sections completed, to the class teacher. The class teacher will return the form to you once completed.

We aim to process all requests within 5 working days of the request, but in busy periods, this may take up to 10 working days.
